Our expert CCM team, led by Rachel Rosenthal, LCSW-C, offers concierge case management services for families in need of additional support.

- Helping you turn testing or treatment recommendations into a plan that is right for your child (and that won’t exhaust your family)
- Helping you make sense of your child’s diagnosis, and consider the right balance between therapy, treatment, family time and leisure
- Helping you select which therapists/professionals are the best fit for your family
- Contacting potential therapists/professionals to determine if they have appts that work with your family’s schedule
- Booking initial appointments with your child’s therapy/educational team
- Helping you prioritize your ‘to do’ list
- Coordinating care across your treatment and educational teams
- Updating clinicians and educators about your child’s changing needs
- Scheduling tours with schools or therapy/treatment centers
- Researching options for therapy/camp/tutors/test prep/aids
- Helping you advocate for your child’s needs
- Help securing the right medical care team to best support your child’s needs
- Helping you select the social and leisure activities that are the best fit for your child’s strengths/support needs
- Contacting therapeutic treatment centers in the DC Metro area to determine if they have space for your child (this service is limited to day treatment partial hospitalization and therapeutic schools in Montgomery County, DC, Fairfax and Loudon Counties)
- Providing a source of calm wisdom and expert advice when needed
